Andover After School Program & Andover Five Alive Program
Enrollment
To enroll in either the Andover After School Program or the Andover Five Alive Program you will need to complete the following forms:
- Registration Form - After School Program
- Registration Form - Five Alive Program
- State Emergency Form
- Health Form:
- Immunization (copy of school forms)
- Physical (within last two years)
- Annual Registration Fee of $30 ($50 per family)
These forms can be downloaded as pdf files, printed out, and mailed to us, or you may telephone us and we will send them to you
